    Looks like a Creme minnow to me. If not they sell one almost identical allready rigged on a hook just like your pic.

    I picked these up at Dick's a couple of years ago. They do very well on a 1/16 oz. jighead and a slow roll retrieve. They are sold by Lindy/Little Joe and are called their "Mighty Minos". Here is a picture:
