I have seen some anglers use those return address labels that you get in the mail. They tape them to the rod in an area where they are not in the way.

Here in Illinois we are allowed 2 poles on a license. A 3rd can be used if it has your i.d. on it. For those of you in the same situation... how do you attach the id to it. I'm thinking small plastic or metal tag attached with small zip tie. Any better ideas found?

I would use a decal on the rod blank. Of course I make decals for a-living so .... kind of cheatin.

In MO, we have the same issue - but have to label anything over 3 rods. We can use name/address or conservation ID#. I use a label maker to make a label with my cons #. I stick it to the pole, then cover it with clear, water-proof Gorilla tape. We do this for spider-rigging and when chasing cats with homemade limb lines.
