We use the ones you shake to activate. We buy them at the end of Winter when they're cheap, nice when it's cold outside.
I used this last week and put a heated hand warmer inside and this was great

Good Idea for those cool cold days. I use Thermal care wraps on my back to keep it loose on cool cold spring days.
first time I did that was in colorado wading in a cold river. I had back spasms so I had one of those wraps on.
Man, I was toasty warm! I guess it really made a difference having your 'core' warm.
My wife was downriver a bit and was freezing. It actually took us a while to figure out why I was so comfy cause I did it for my back... and not to stay warm.
Now I also use hand warmers and "Toasti Toes" or hot hands toe warmers in my boots. It's just a small air activated packet that you stick to your socks.
