All Pre-Tournament meetings begin at 6:00 P.M. with a free meal for participants at the
Carlyle and Rend tournaments. Attendance at meeting is not required for tournament
teams. ALL TEAMS MUST BE SICA MEMBERS. Cost to join the SICA is $15.00. You
may use this form to join the Sica, Inc. Entry Fee: $100.00 per team per tournament
includes Big Fish Pot of $10.00 per team - Make check payable to S.I.C.A. and mail to

ALL TEAMS MUST BE SICA MEMBERS. Cost to join the SICA is $15.00. You may use
